We fixed the mating sleeve on the 6-Port FHD multimedia modular panel. Then the connection goes like this: 100G QSFP28 SR4----MTP Female 12 Fibers Type B----fiber optic mating sleeve----MTP male to 4 LC UPC Duplex 8 Fibers Type B----25G SFP28 SR
The modular panel is good for the sleeve, in case you leave the adapter in random place.
